#2710 gatorwalk-factory: publish explains a stuck status step, and every refused lifecycle copy (GW-17 follow-up)

Follow-up to #2685 (the projection publisher), from its verification reviews. None of these change what publish delivers; they make failures easier to act on.
- A status step that always fails holds the cursor, and so every later comment, until someone acts. This happens when the current stage's status key is missing from the adapter's statuses argument, or the ticket is in a status the adapter does not know. That is by design (report, do not skip), but the error should say what to fix (the key and the statuses argument to add it to) and that new comments wait on it. Consider whether comments should still be delivered and the cursor advanced for them, with only the status step retried.
- When every candidate copy of the pinned lifecycle is refused (the version query and the latest copy), only the last refusal is reported. Report each candidate's reason.
- Whether a past advanced event is worded as entered or finished is decided from the lifecycle pinned now, so after a repin an old event can be worded from the newer lifecycle. Cosmetic; decide whether to word from the lifecycle pinned at that event, or leave it and say so in DESIGN.md.

One more from the final verification review of #2685: an approval comment includes the actor's platform principal as it is, while projection.ts says every value in a comment body is a NameSchema name. A principal holding characters swamp-club refuses (two adjacent double quotes, or a leading dollar sign, per swamp-club#2284) would make the Lab refuse that comment, and publish would stop at that event on every re-run. Either restrict the principal to a safe alphabet (and name the actor as unidentified otherwise), or correct the comment and treat such a refusal like any other stuck delivery.
