#499 support git forge / web namespaces for collectives

Today extensions resolve only through the central swamp.club registry. It would be great if a collective could be backed by a web / git forge namespace, so a team can pull extensions from their own forge instead of the central registry.
I see #12 (private extensions) was closed recently. Was there a reason? This may overlap with that need, so I wanted to check whether it was declined, deferred, or superseded before pushing on it.

@nblair2 one more addition here - https://swamp-club.com/manual/how-to/extensions/use-local - you can check out a repo and point to that repo using the guide above
