The token management table in .claude/skills/swamp/references/serve/guide.md (line 188) lists SWAMP_SERVER_TOKEN as the env var for server access tokens but does not mention that SWAMP_SERVER_URL must also be set for the token to be sent. Users reading the guide would set SWAMP_SERVER_TOKEN alone and get a silent failure (now a warning after swamp-club#2325). The table should add a note that SWAMP_SERVER_TOKEN requires SWAMP_SERVER_URL to scope which server the token applies to.
