integration/access_poll_phase_telemetry_test.ts:473 failed once on macOS in the scheduled flaky-test run https://github.com/swamp-club/swamp/actions/runs/40743652026 (commit 992330ba0): the local change source test expected grants_active 2 and got 1. The two assertions just before it passed, so records_listed and records_read were both 2. Nothing yet shows this is a timing assumption in the test: a record that was read but is missing from the active grants may be a real defect in src/domain/access/policy_snapshot_loader.ts, or a read that saw a partly written record. Needs its own investigation. Found while triaging #4160.
