src/domain/extensions/local_model_refresh_test.ts:492 (refreshLocalModels: a broken model warns once, not on every later refresh) has failed on 25 Windows Compatibility runs since 2026-10-31 08:07Z (latest https://github.com/swamp-club/swamp/actions/runs/41864132261, dd6591148):

+ 1A broken repo-local model warns twice on Windows, where it should warn once. The test dates from 68e0ed7d5 (swamp-club#3284) and passed on Windows until the range 345cd3673..e459d219f. The likely cause is 1a4e8b36d ("fix(serve): reconcile externally indexed local extensions"), which changed the refresh. 9e1eb6404 (an explicit reload forces the refresh past its unchanged-files check) has touched the same file since.
Probably the "already warned" key does not match between refreshes on Windows. Possible causes are a path with different case or separators, or a catalog row whose stored path differs from the scanned one. Either way the second refresh warns again. Serve users on Windows would see the warning repeat.
