#3422 CI adversarial review gate keeps blocking on a stale CHANGES_REQUESTED review when the new review's heading is formatted differently

The 'Fail if changes requested' step of the Adversarial Code Review job selects the last github-actions review whose body matches ^(## )?Adversarial Review. On PR 3091 (swamp-club#3420) the review of the new head 8e332c46 began with 'Adversarial Review' and a PASS verdict, so it did not match; the gate picked the earlier CHANGES_REQUESTED review on fdb409e4 and failed the check, blocking auto-merge on a passing review.

Expected: the gate reads the verdict of the review for the current head commit (filter on commit_id), or matches the heading regardless of markdown emphasis, so a stale review on an older commit can never block.
