Follow-up to #2770 (PR #396), from its verify-reviews adversarial review (low).

The plan stage injects plan-feedback, which resolves to the latest record in the era. On the path revise -> plan -> submit -> plan-review (open high finding) -> rework -> plan, that later plan pass is handed the same feedback it already answered. The plan prompt in all three example factories (swamp-club-swamp-extensions, build-swamp-extension, starter) calls it the person's latest feedback and says to record feedbackIncorporated as the last plan's list with the latest feedback added, so the planner can list the same round twice, and plan_revised then shows it twice in the Lab.

Fix: the prompt carries the last plan's list forward and adds the feedback only when it is not already the list's last entry. Prompt-only; no engine change.
