While running the issue-lifecycle for swamp-club #2620, approve succeeded locally (phase moved to approved, and implement and verify worked afterwards) but logged: swamp-club patch failed: 422 Cannot triage issue in status triaged, Expected open. It looks like approve sends a triage status transition for an issue that the triage method already moved to triaged. Either the transition is wrong for this phase, or approve should treat an already-applied status as a no-op, as the skill says re-runs do.
