#892 docs: update swamp-serve manual reference for phase 2 --server commands

Context

#711 added --server support to ~35 additional CLI commands (phase 2). The manual reference docs in swamp-club/swamp-club content/manual/reference/swamp-serve/ should be updated to document the full set of remotely available commands, authorization requirements, and commands excluded from remote execution.

Changes to document

Full list of --server-capable commands across data, model, workflow, vault, extension, doctor, and server admin categories
Authorization levels per command (read/write/run/admin)
Commands not available remotely: doctor audit (requires local process execution), batch data delete --prefix/--all (requires interactive confirmation)
New streaming frame type: workflow.resume
6 server handlers that return not_implemented: datastore.status, extension.search/install/rm/outdated, doctor.extensions
