#946 Docs: document global skill sync behavior for custom tools in manual

Context

PR #1743 (swamp-club#943) changed how swamp update and repo init/repo upgrade handle global skill syncing for custom tools:

swamp update now only syncs to built-in tool directories that already exist on disk
Custom tools with ~/-prefixed skillsDir get bundled skills installed globally and kept current via custom-tool-skill-dirs.json registry

What needs updating in swamp-club/swamp-club

Add a section explaining:

Custom tools with ~/-prefixed skillsDir get bundled swamp skills installed globally (same as built-in tools)
Custom tools with repo-relative skillsDir (no ~/ prefix) get skills installed per-repo only
swamp update automatically keeps global custom tool skill directories current
The ~/.config/swamp/custom-tool-skill-dirs.json registry bridges the gap between repo-scoped config and global swamp update

May want to mention that autoupdating now respects enrolled tools — it won't create skill directories for tools the user hasn't set up.
