Parity with #1341 (datastores). The three vault extensions (aws-sm, azure-kv, 1password) make SDK calls that are invisible in traces. Add OTel spans for vault get/set/delete/list operations so failures and latency are attributable in the trace. Same approach as the datastore work: @opentelemetry/api only, no-op without a TracerProvider, follow OTel semantic conventions where they exist.
