Context

swamp-club/swamp#1776 adds SWAMP_SERVE_EXTRA_HEADERS — an environment variable that lets swamp clients send custom HTTP headers when connecting to swamp serve through reverse proxies or tunnels.

Requested by swamp-club#990.

Documentation gaps

The following manual pages need updates:

tls-and-proxies.md (reference) — Add a section on configuring extra headers for reverse proxy authentication (e.g. Tunnel-Token). Cover the env var format (newline-separated Name: value), reserved header names, and secret handling.
swamp-serve.md (reference) — Mention SWAMP_SERVE_EXTRA_HEADERS in the client configuration section alongside SWAMP_SERVE_URL and SWAMP_SERVER_TOKEN.
how-to/swamp-serve.md (how-to) — Add a how-to example for running swamp serve behind a reverse proxy that requires custom headers.
