#2127 Docs: access-commands.md still describes old vault-storage behavior for token mint

The manual reference at content/manual/reference/swamp-serve/access-commands.md still says "The plaintext is stored in a vault" for access token mint, lists --vault as an option without noting it is rejected when a datastore is configured, and doesn't mention the control-plane vault override.

Fixed in CLI help text and serve skill guide in #2116. The manual should be updated to match:

  1. Remove "The plaintext is stored in a vault" from the mint description
  2. Note --vault is only for local repos without a datastore
  3. Add that swamp access token reveal is the retrieval path when a datastore is configured
