Follow-up from #2094. The workflow execution report now includes approval metadata, step outputs, and startedAt/completedAt timestamps. UAT coverage is needed for: (1) history get JSON output shape with new fields, (2) the new swamp workflow history outputs command (not yet implemented). Tests belong in swamp-club/swamp-uat under tests/cli/workflow/history/.
